By signing this, you understand that there are some risks involved, like using art supplies and tools, physical movement and interaction with other participants. We promise to keep things as safe as possible! *

2- You’re giving us the thumbs-up to take good care of your child. If your child gets hurt, you’re okay with us helping out and even getting medical help if needed. (Don’t worry, we’ll call you right away!) *

By checking this box, I authorize the Studio, in the event of an emergency, to seek medical treatment for my child. I understand that I will be responsible for any medical expenses incurred.

3- We want everyone to have fun, so we ask that all children follow our studio rules and expectations. If someone’s having a hard time following the rules, we might have to call it a day for them. *

I understand that my child is expected to conduct themself safely according to the Petite Art Studio expectations and I understand that the Studio reserves the right to dismiss any participant whose behavior is deemed inappropriate. This includes, but is not limited to: anyone who disrupts a program or anyone who fails to abide by the Studio’s rules and practices.

4- We love capturing the magic of our art adventures! By signing this, you’re okay with us taking pictures and videos of your child to share the fun on our website and social media. (No worries, it’s all in good taste!) *

By checking this box, I grant permission for the Studio to take photographs and/or videos of my child during activities for promotional and educational purposes. I understand that these images may be used in various media, including the Studio’s website and social media platforms.
